Date Formatting in Google Form

Google Form is a great app for making interactive form to be inserted in a website or blog. Google Form provides "click and select" input options to make the creation of the form's inputting fields a bliss. If a form needs a field to collect "Date", it's only one click away to insert. Unfortunately, the defaulted format of the "Date" field is mm-dd-yy. If you want just the month and year or other formats, it can't be done. There is no solution for it as of now. For those of us who have yet to learn to code our own interactive form, we just have to put up with this. In my case where I need only the year and the month, I just have to do it by inserting two separate questions.
